I AGREE TO:

Meet the entry requirements and provide previous qualifications prior to the course.
To work safely within the boundaries of my course.
Arrive on time at the beginning of the day.
Use courteous, polite language and treat staff, tutors, models and fellow students with respect, dignity and equality.
Follow the health and safety provided at the time of my course to protect others and myself.
Not to attend this course under the influence of drugs or alcohol.
Dress appropriately and smart as per course overview.
Not to copy or resell any study materials provided.
To only practice in the way I am taught by Ampika’s Aesthetics and not to practice outside the boundaries set.
Seek and gain insurance before carrying treatments once qualified.
My details will be stored by Ampika’s Aesthetics under the data protection act (2018).

I HERBY AGREE TO THE ABOVE AND UNDERSTAND THAT BREAKING THIS CONTRACT MAY RESULT IN MY REMOVAL FROM THE COURSE.
