IMPORTANT INFORMATION FOR PRESCRIPTION ORDERS

Please note that each prescription of up to 250 units of any Anti Wrinkle injectable order MUST BE FILLED OUT as a medical consultation form that we provide on each order. This consultation form is required to enable your medical prescriber to view your clients suitability to having the medical treatment carried out.

The consultation form is YOUR CLIENTS information NOT YOUR INFORMATION. If you are ordering for multiple clients then you must complete an order for each one.

Once you have selected your anti wrinkle injectable product, filled out your clients medical consent form and purchased your products you shall receive your items in 3-4 working days.

All products are tracked and delivered via DPD so you will be contacted via text or email to track and receive your parcel.
We hold no responsibility of the product’s availability or delivery as this is outsourced to a third party.

All private scripts raised are processed through Ampika’s training Academy. All orders are made payable through the website this is not a telephone based service.
